Welcome to MBC's Vacation Bible School!

We are so excited you will be joinin us for a week full of God's word, fun, games, crafts and friends! Please register your child (one form per child) using this form. This event is free and kids from ages 4 to 12 are welcome to attend.

I grant permission for photo(s) of my child/children to be used for publicity in promotional purposes, which include in-house presentation, the Midvalley Bible Church website, brochures, and newsletters. further, I understand that no personal/identifying information of my child will appear with photos available to the public.

TERMS AND CONDITIONS

I understand that my child/children may participate in physical activities such as those held during game time. As with any physical activity, I understand that there is a risk of injury. I fully accept this risk and hold harmless from any legal liability, Midvalley Bible Church, it's representatives and any persons involved in the Vacation Bible School (VBS) ministry.

In the event of an emergency that requires medical treatment for the above named child/children, I understand that every effort will be made to contact me or my emergency contact. However, if I/we cannot be reached, I give my permission to the VBS volunteers to secure the services of a licensed physician to provide the care for my child's well-being. I assume full responsibility for any and all costs, medical or otherwise, incurred in connection to any accident or treatment of my child.
